2024 annual report shows how FQHCs are succeeding in value-based care with Yuvo Health

2024 was a big year for FQHCs and value-based care. 

Until now, VBC has delivered more work to FQHCs’ already over-stretched teams, with no reward. But we’re flipping the script. You can read all about it in our latest annual report: “An ecosystem of support for community health centers.”

This year, Yuvo Health was proud to announce that our FQHC partners officially earned shared savings in just their first year of our value-based contracting program and are also earning monthly, upfront payments as part of their VBC contracts through Yuvo. Just as important, our partners are receiving hands-on help, guidance, patient insights, technological capabilities, and quality improvement support from Yuvo as much-needed relief for their teams.

With this comprehensive support, our FQHCs have achieved an overall 65% improvement in quality performance according to all metrics where we have complete data for 2022-2023, including Breast Cancer Screening, Colorectal Cancer Screening, Immunizations for Adults and Postpartum Care.  

We have always known that FQHCs are uniquely positioned to advance the value-based care objectives of delivering higher quality care at a lower cost. With this early success, we can now spread that message far and wide at an opportune time when collaboration has emerged as a major theme in community health. 

For example, the NYS 1115 Medicaid Waiver Amendment, also known as the New York Health Equity Reform (NYHER), represents a huge opportunity to reshape social care delivery in New York. FQHCs should be at the center of these initiatives.

Technology powers a leap forward for FQHCs

Health technology is often rife with information gaps, months-long lag times for critical data and extra work for providers. That’s why we have focused on developing and deploying solutions that offer a comprehensive view of a patient’s health history in a way that is easily accessible, right at the point of care. We have already introduced this capability to several of our FQHC partners and are actively deploying it with others.  

This year we have also introduced new text messaging capabilities to aid in patient engagement and deployed a data warehouse that empowers FQHCs with the clarity and confidence to assign patient data from various sources to the right person.

Soon, we are excited to enable FQHCs with greater care management opportunities by providing real-time access to admission, discharge and transfer data and to make the data even easier to understand with enhanced visualization.

FQHC spotlight: seamless tech integrations equip providers at the point of care 

With technology provided by Yuvo Health, Joseph P. Addabbo Family Health Center (Addabbo) providers and team members can now view claims data, patient health history, admissions, discharges and transfer (ADT) data and more from multiple sources directly in their EHR.

This equips the Addabbo team with greater insights into open care gaps and suggested opportunities to close those gaps to drive quality improvement. Plus, automated documentation relieves the team of substantial administrative work.  

Now, providers can maximize their time with the patient and as a result, the Addabbo team can accomplish more in less time.
